Hillside Primary Care, PLLC, doing business as Hillside Medical Group (“Hillside,” “we,” “us”). These Terms & Conditions govern use of our website and our patient SMS program.

1. Agreement

By using hillsideprimarycare.com, related clinic websites, patient portals, or by opting in to SMS messages from Hillside, you agree to these Terms & Conditions. If you do not agree, do not use the site and do not opt in to SMS.
Medical care is provided under a separate treatment relationship. These Terms do not replace a Notice of Privacy Practices, informed consent for treatment, or any other clinical form.

2. SMS Terms

SMS program terms

This SMS program is operated by Hillside Primary Care, PLLC d/b/a Hillside Medical Group.

Program / message types. After you opt in, we may send appointment reminders, schedule-change notices, telehealth visit reminders and access instructions, patient-portal or intake notifications, and billing or statement reminders. Messages do not include diagnosis or test results. This program is not used for third-party marketing.

Who messages come from. Messages are sent by Hillside Primary Care, PLLC d/b/a Hillside Medical Group. Related practice names that may appear in clinic materials include Psychiatry of SA, Women’s Wellness of SA, and Podiatry of SA. SMS under this campaign is sent as Hillside Medical Group.

Opt-in. You opt in only if you:

  • check an unchecked SMS box on a web or paper intake / scheduling form, or
  • give verbal consent using our staff script, after which we send a confirmation text.

Consent is not required to receive care and is not a condition of scheduling or treatment.

Frequency. Message frequency varies and is tied to scheduled visits and account activity.

Rates. Message and data rates may apply.

Help. Reply HELP or call 210-742-6555 or email office@hillsidemedicalgroup.com.

Opt-out. Reply STOP, STOPALL, UNSUBSCRIBE, CANCEL, END, or QUIT. You will receive a confirmation and no further program messages. You may also ask the front desk to remove your number.

Carriers. Wireless carriers are not liable for delayed or undelivered messages.

SMS data. We do not sell or share your SMS opt-in data or personal information with third parties for marketing purposes.

3. Website use

Site content is for general information about our clinics and services. It is not medical advice and does not create a provider–patient relationship. Do not use the website for emergencies. Call 911 or go to the nearest emergency department.
You may not misuse the site, attempt to access systems you are not authorized to use, or submit another person’s information without authority.

4. Privacy

How we use and disclose protected health information is described in our Notice of Privacy Practices:

5. Appointments and financial policy

Scheduling, no-show, cancellation, insurance, and payment rules are set out in the patient intake packet you sign at the clinic. Those clinical and billing forms control if there is a conflict with this web page, except that SMS consent is governed by the SMS Terms in Section 2.

6. Changes

We may update these Terms. The current version will be posted at this URL with an updated effective date. Continued use of the website or continued participation in the SMS program after an update means you accept the revised Terms. You may opt out of SMS at any time by replying STOP.
