Hillside Primary Care, PLLC, doing business as Hillside Medical Group ("Hillside," "we," "us"). Effective: October 8, 2026.

This Privacy Policy explains what information we collect through hillsideprimarycare.com and our text message (SMS) program, the Hillside Medical Group Patient & Care-Team Text Messages program, how we use it, and how we protect it. How we use and disclose protected health information is described in our Notice of Privacy Practices.

Information we collect

• Contact details you give us, such as name, mobile phone number, email address and date of birth, when you register, book an appointment, complete a form or agree to receive text messages.
• Your text message consent record: the date, the staff member who recorded your verbal consent, and the mobile number enrolled.
• Basic website usage data, such as pages visited and browser type.

How we use your information

• To send the text messages you agreed to receive: appointment reminders and confirmations, schedule changes, check-in and telehealth visit links, patient-portal and intake-form notices, and billing reminders.
• To respond to your questions and process HELP and STOP requests.
• To operate and improve our website and services.

Text messaging (SMS) and mobile information

Your mobile phone number is used only to send the text messages you consented to. Message frequency varies. Message and data rates may apply. Reply HELP for help or STOP to opt out at any time. Consent is not a condition of receiving care.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors in support services, such as customer service, is permitted. All other categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

How we protect your information

We use administrative, technical and physical safeguards consistent with HIPAA.

Your choices

You may opt out of text messages at any time by replying STOP, by asking our front desk to remove your number, or by contacting us below. You may also contact us to review or update your information.
